# Service animal to the United States

Traveling with a service dog to the U.S. requires advance preparation. To ensure your service dog can travel with you, all required documentation must be completed and submitted to us at least **48 hours before departure**.

It is the passenger's responsibility to ensure all required documentation for both the country of departure and destination is complete and available at check-in.

**Please remember to bring all original documentation with you to the airport for verification.**

### Required Documentation

To start the approval process, please contact our Customer Care team through our chatbot or email support@flynorse.com.

We will send you the following forms to complete and return:

*   Acceptance of Service / Assistance Dogs Checklist for USA Arrivals
*   U.S. Department of Transportation Service Animal Air Transportation Form\*
*   U.S. Department of Transportation Service Animal Relief Attestation Form\*

_\*Air Transportation and Relief Attestation forms must be done per flight for round-trip journeys._

In addition to the forms above, you must provide:

*   CDC Dog Import Form Receipt

Depending on where your dog is traveling from, additional documentation may also be required, including:

*   Certification of U.S.-Issued Rabies Vaccination
*   USDA-Endorsed Export Health Certificate
*   Certification of Foreign Rabies Vaccination and Microchip
*   Rabies Serology Titer from a CDC-approved laboratory
*   Confirmation of a reservation at a CDC-registered Animal Care Facility
*   Any additional documents specified in the USA Assistance Dogs Checklist

### CDC Dog Import Form

All dogs entering the United States must have a **CDC Dog Import Form Receipt**.

To obtain your receipt, complete the official CDC Dog Import Form on [official webpage](https://www.cdc.gov/importation/dogs/dog-import-form-instructions.html) and select "Service animal" where applicable when completing your application.
